We created our methodology for reducing human error by studying real-life incidents. It began with Larry Wilson, the author and founder of SafeStart, YOUFactors’ parent brand. Increasingly aware of human factors in safety management, he talked to thousands of people in various industries, conducting hundreds of observations and – in cooperation with his clients – analysing more than 400 incident and accident reports.
All his behavioural safety efforts targeted a single objective: Find the missing pieces in the puzzle of how incidents and accidents really happen – and develop tools and techniques to interrupt avoidable domino effects. Initially, Larry focused on improving behavioural safety and performance in the workplace but, over time, he included life outside it too.
